Shift rounds, readings and process conditions, side by side.

The operator runs the round on a phone. When a reading falls out of range and an observation is recorded, the operations supervisor raises a process condition with “elevated” attention, and it shows on that unit of the plant.

01 · Rounds and checklists

Shift rounds from a published routine.

A process routine sets what is read, where, and in what range on each shift. Each round is open, in progress or completed, and overdue is worked out from the time.

02 · Readings and trends

Every reading with its time, source and expected range.

Readings, manual or from a data source, belong to a measurement point. A recorded reading is never deleted; a correction is a new record next to the old one.

03 · Process conditions

An attention level, and a path to resolution.

A process condition is raised on a unit or stream with routine, elevated, heightened or critical attention. It is active, being monitored, or resolved, and every change keeps its name and time.

04 · Actions and outcomes

What was done, and what it changed.

Operational actions and outcome assessments are separate, lasting records. Six months later you can still see what you did about the oxygen drop, and whether it worked.

05 · From observation to repair

An observation that becomes a repair request keeps its context.

When the operator sees the problem is in the equipment, the observation becomes a work request. The unit, the reading and the photo stay attached, so the maintenance supervisor doesn’t have to ask again.
